This is a pre-registration form for Winter Small Group Training.
It is for us to collect your information and contact you for your evaluation and group placement. After the evaluation session you will find out what group, day & time you'll be placed in.
Groups will be on Wednesdays and or Thursday between 5:30pm & 8:30pm. You will be placed in a group based on your availability and skill level of the group.
